Arrange to have official health records sent to the Flathead Valley Community College Admissions & Records office.

• Please provide office health records from doctor, school or health clinic showing dates for TWO MMR immunizations (measles, mumps, rubella).
  
  
• FIRST immunizations must have been given AFTER first birthday.
  
  
• Any immunizations received prior to 1968 are considered invalid.
  
  
• This requirement applies to anyone born on or after January 1, 1957, who is in degree/transferring status or taking 6 or more credits.
